  • Contact Information: Name, mailing address, email address, and mobile phone number.
  • Plan Data: Enrollment status, group numbers, and claim history.
  • Protected Health Information (PHI): As defined under HIPAA, we collect information regarding your healthcare services and payments.

Self Fund Health uses SMS messaging to provide enrolled members with timely updates regarding their health plan. By providing your mobile number and opting in to our messaging program, you agree to the following:

  • Purpose of Messages: We send informational messages including benefit reminders and program notifications.
  • Consent & Opt-In: SMS consent is obtained directly from you during plan enrollment or via our member portal. Mobile information (phone numbers) and SMS consent will not be shared with third parties or affiliates for marketing or promotional purposes.
  • Opt-Out: You may cancel the SMS service at any time. Just text "STOP" to the short code or long code from which you received the message. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed.
  • Help: If you are experiencing issues with the messaging program you can reply with the keyword "HELP" for more assistance.
  • Carrier Disclosure: Message and data rates may apply for any messages sent to you from us and to us from you. Message frequency varies based on your account activity. Carriers are not liable for delayed or undelivered messages.

3. How We Use and Share Information

We use your information solely to conduct "Health Care Operations" and "Payment" activities as defined by HIPAA, including:

  • Processing and paying health claims.
  • Responding to member inquiries.
  • Coordinating care and disease management.

No Sale of Data: We do not sell, rent, or lease member lists or personal data to third parties. We only share data with "Business Associates" who are contractually obligated to protect your data under a signed Business Associate Agreement (BAA).

We implement physical, technical, and administrative safeguards, including encryption and secure access controls, to protect your data within our systems and during transmission.
