How Do I Transfer My Medical Records from My Current PCP to My DPC Using MyChart?

If your current provider uses MyChart (or a similar electronic health record portal), you can usually share your records directly with your new Direct Primary Care (DPC) provider by generating a secure link. This can be faster than requesting paper or faxed records.

Steps to share your records through MyChart:

  1. Log in to your MyChart account.
  2. Go to the Health or Medical Records section.
  3. Look for an option such as “Share My Record,” “Download My Record,” or “Share Everywhere.”
  4. Choose to generate a share link or access code.
  5. Send that link or code to your DPC’s office by email or secure message, or give it to them over the phone so they can retrieve your records.

Tip: Some DPCs prefer fax or secure email for compliance reasons. If that’s the case, you can still use MyChart to download your records as a PDF and send them to the DPC directly or ask your current provider to fax them on your behalf.

Requesting and sharing your records early — ideally soon after you receive your DPC assignment — helps ensure your new doctor has everything they need by your first visit.
