How Do I Download an Explanation of Benefits (EOB)?

An Explanation of Benefits (EOB) shows how a claim was processed, including what the plan paid and what you may owe.

Steps to download your EOB:

1. Log in to the Member Portal.
2. Go to the Claims tab.
3. Select the claim you want to review.
4. Click Download Explanation of Benefits at the bottom of the page.

Your EOB is the best way to confirm that your provider’s bill matches what your plan covered.
