Why Did I Receive This Bill From a Provider?

Sometimes a bill looks higher than expected because the insurance discount wasn’t applied yet.

Steps to take:

  1. Look closely at the bill to see if an insurance discount/adjustment is shown.
  2. If no discount is listed, call the provider’s billing office and confirm they have your current Self Fund Health insurance information from your ID card.
  3. Once corrected, the bill should reflect the discount.

If it still doesn’t look right:

  • Compare the bill with your Explanation of Benefits (EOB) in the member portal.
  • If the provider’s bill and your EOB don’t match, contact the Plan Concierge at 608-506-1336 or questions@selffundhealth.com.
