How to Change Your Direct Primary Care (DPC)?

Your Direct Primary Care (DPC) provider—sometimes called your Primary Care Provider (PCP)—is your main point of contact for everyday healthcare needs. If you’d like to change your DPC, it’s simple.

Steps to change your DPC:

• Contact your nurse navigator at nurse@selffundhealth.com or call 608-284-8831.
• Let them know you’d like to switch to a new DPC provider.
• Your nurse will help you find an available DPC in your area and update your assignment.

Once the change is made, you’ll use your new DPC for ongoing care, referrals, and access to no-cost labs and medications.
