How To Identify Networks on ID Cards?
Members receive ID cards with the logo of the network(s) that apply based on where they reside.
- A Wisconsin member may see The Alliance, Trilogy or The Alliance/Trilogy combination network logo on their card.
- An Iowa member will see Midlands Choice network logo.
- A Minnesota member will see America’s PPO network logo.
- Out-of-the Service Area or Out-of-State members should refer providers to the First Health network logo.
This ensures providers can quickly identify which network to bill, while members get seamless access across regions.
TIP: When a provider asks who the insurance is, the member say the applicable network that is listed on their ID Card. If they are outside of the primary network service area or out of state, they need to say "First Health Network" as this is the wrap network that provides nationwide in-network coverage.
